#307 Sequence/Dynamic diagram - simplify to "one" Rel() call

- Sequence diagram
  - Add SHOW_INDEX() layout option
  - Rel() extended with $index, $rel (replaces RelIndex() and RelSpecialIndex())

- Dynamic diagram
  - Rel() extended with $index (makes RelIndex...() obsolete)
